      I am not liable for this debt with ****** PERMANENTE SCAL. I do not have a contract with USCB America, they did not provide me with the original contract as requested. Instead I received a notice from USCB America with inaccurate information on the notice.

      A medical debt from ****** Permanente was sold to debt collectors which has a balance of less than 500$. The amount currently is $474. Per the credit bureau's recent rules and the consumer financial protection bureau's recent instructions to debt collection companies, which is not to furnish medical debt less than 500$ to the credit bureaus, this account should not have reported. I also have not entered into a contract with USCB America and have not furnished that to me as requested.

      I received a deb ********* **** for $690.66 from this company. I called and spoke to them and stated I should never have been ****ed because I have ******** and UMR. UMR was ****ed and the agent confirmed ******** was not. I advised her that my SS# is my Mediciad Number and I have no ******** card because Colorado doesn't send them out. She demanded I pay and ask for reimbursement through ******** myself. I advised her she needs to reach out and **** ********. She would not help me.

      I believe this account is reporting inaccurately. I do not recall having an account with this company. I don't think there is legal contract between this ***************** I would like you to provide me verifiable proof this account belongs to me.       Between May 31st and June 1st of 2023 I received a collection notice from uscb America of an alleged debt of $88.70 I have no knowledge of this alleged debt. I responded to these allegations June 15th 2023 uscb America is in receipt of my response in the same month. I have a certified receipt as proof. In accordance to us code 15 section **** g I demanded a hard copy of the original signature of validation I never received such I did however receive a document with a signature that does not belong to me further uscb America is in violation of the ***** act by furnishing a document of my past visit to ****** Permanente.

      ****** sent 2 bills in total for $1055.74 to **** America Debt collections 1. one dating back to 2020--I am only now being made aware of in excess of $950 with interest -the attachment labeled GCerruti.Account.summary is comprised of multiple charges, however, it is a collective bill for ****** labs requested by provider not individual office visits. 2. and an another for $58.59 for 9/2022 approx.The older bill was for lab fees for a procedure the PROVIDER decided was too extreme given condition only after REQUIRING the labs and the second bill for $58.59 was for a telephone doctor's *********** in which the physician NEVER called. Both of these bills I contacted and spoke to ****** about and they stated were resolved after I filed a grievance given the aforementioned reasons. However, instead they were sent to collections. I would greatly appreciate assistance with the following:1. contacting **** America in order to assist with 2. the bills reflect an appropriate balance of zero amount owed 3. and my credit report corrected for both erroneous delinquent unpaid charges.

We are filling this complaint against USCB America on behalf of our client, *******************. This should not be confused with the complaint we filed against USCB America on behalf of another client, ******** Weldegherghis.
